Using standard of substituted judgement, the parents do what they believe the children themselves would want. As fetus’s opinion is unknown, all we can do is fallback on the basic assumption that sentient being have an interest in avoiding pain.

For the patient in a irreversible unconsciousness, it is hard to use Personal Liberty or pain avoidance to justify the euthanasia. For this point, there really isn’t any personhood left.
